Plate on control box reads "MIDDLEWEST INSTRUMENT CO./ SERIAL NO./ 25 CYC./ CYC. 60 ALTERNATING 110 V.". Decal on control box reads "AMERICAN/ MEDICAL/ ASSN./ ACCEPTED COUNCIL/ ON PHYSICAL/ THERAPY". Decal applied to frame reads "DO NOT ACCEPT/ THIS UNIT/ AS A NEW MACHINE,/ UNLESS ACCOMPANIED BY/ SERIAL NUMBER CARD FOR/ REGISTERING IT WITH US." "10106" incised in frame, above .2 tank.
